Online:

Visit the NHS website dedicated to data matters: www.nhs.uk/your-nhs-data-matters.
Follow the instructions to set or change your opt-out choice.
You will likely need to provide your NHS number or postcode and a mobile number or email address registered with your GP.

Through the NHS App:

Open the NHS App.
Navigate to “Your Health”.
Select “Choose if data from your health records is shared for research and planning”.

Non-Digital Options:

For individuals who cannot access the online service or the NHS App, there are options to opt-out via phone, email, or post.
You can find details on how to manage your choice through these channels on the NHS website.

Important Notes:

The National Data Opt-out service was previously known as the Type 2 opt-out.
This opt-out applies to the use of your data for purposes beyond your direct care, such as research and planning.
If you have children under 13, parents or legal guardians can set the opt-out on their behalf.
For individuals with a formal proxy relationship, like a lasting power of attorney, they can also set the opt-out on behalf of the adult they represent.
If you need to opt-out on behalf of someone else, you may need to complete a specific form and potentially provide documentation to verify your relationship.
